Ok Ampers it is time to put your thinking caps on, and make sure they are turned to "evil". Let's get a list of ideas that would be cool to see in HJ in relation to world design. One main focus would be phobias. I would also love to see some ideas on other fun ways to torture heroes.
So come on, tap into your evil side (I know it is there!) and get some ideas down!

Rats. Rats are always great. Any rock you lift or door you open can bring out a rat that will immediately jump on you. It is probably not strong enough to kill you. But if it scares you enough times, you will stop opening crates or trapdoors.
Also fun are the headless bomb men from Serious Sam. You hear them shouting from far. So you know they are coming. But to freak a party really out is if they are not always coming.
Maybe a nasty domestic problem. Your house can be invested my termites. Not sure how to prevent it, or how you it will make you paranoid. But maybe leaving your house empty for too long or something.

Walking in a narrow cave and hands are coming out of the wall to grab you. They might not hurt you but they keep you rooted, and freak you out.
Small poddles of water that are normally just just shallow water. But then all of a sudden they are not and you drop down.
Normally NPC's send you on quests for something noble and dangerous, and you get a reward afterwards. But there will be some NPC's that will send you on a quest that is just evil. No reward, just selfishness on his part. This one needs more thinking, but I am just throwing it out.

Hrm, Evil, Phobia and World Design...
The freakiest thing I've encountered... ever... is ghosts.
In Dragonrealms, there is a manor with a tragic story behind it (designed that way).

That website, although very interesting, does not do it complete justice as to how freaky it gets. Various sounds and screams are heard through the area. If you examine things in detail, you start to pick up on the tragic history behind the manor.
The place is full of undead or ghosts of some type, some of which can kill a party in seconds. It's risky to be there.

My words can not spell out the true nature of this manor and I still get goose bumps thinking about the first time I ever went there (1996).
I'm not world design. But man, it would be fun to code things for this.

Ok now that place would really creep me out. After reading the second story I had goosebumps. Wow, not sure if I could actually bring myself to go to a place like that in a game.
